✔️ Indication of Emeren 8mg Tablet

Emeren 8 belongs to a group of medications called 'anti-emetics primarily used in the prevention of vomiting (being sick) and nausea (feeling sick) that usually occur after cancer chemotherapy, radiation treatment, or surgery.

✔️ Pharmacology

Ondansetron is a potent, highly selective 5HT3 receptor antagonist. Its precise mode of action in the control of nausea and vomiting is not known. Chemotherapeutic agents and radiotherapy may cause the release of 5HT in the small intestine initiating a vomiting reflex by activating vagal afferents via 5HT3 receptors. Ondansetron blocks the initiation of this reflex. Activation of vagal afferents may also cause a release of 5HT in the area postrema, located on the floor of the fourth ventricle, and this may also promote emesis through a central mechanism. Thus, the effect of ondansetron in the management of nausea and vomiting induced by cytotoxic chemotherapy and radiotherapy is probably due to antagonism of 5HT3 receptors on neurons located both in the peripheral and central nervous system. The mechanisms of action in postoperative nausea and vomiting are not known but there may be common pathways with cytotoxic-induced nausea and vomiting.

✔️ Dosage Recommendations for Ondansetron

Chemotherapy-Induced Nausea and Vomiting:

Adults and Pediatric Patients (6 months to 18 years):

• Tablets/Orodispersible Tablets (8 mg): Three doses of 0.15 mg/kg, up to 16 mg per dose.
• Orodispersible Tablets (4 mg): Three doses of 0.15 mg/kg, up to 16 mg per dose.
• Injection: Three doses of 0.15 mg/kg, up to 16 mg per dose, infused intravenously over 15 minutes.

Radiotherapy-Induced Nausea and Vomiting:

Adults:

• Tablets/Orodispersible Tablets (8 mg): 8 mg orally 1-2 hours before radiotherapy, then 8 mg every 8 hours for up to 5 days post-treatment.
• Orodispersible Tablets (4 mg): Three doses of 0.15 mg/kg, up to 16 mg per dose.
• Injection: Three doses of 0.15 mg/kg, up to 16 mg per dose, infused intravenously over 15 minutes.

Postoperative Nausea and Vomiting:

Adults:

• Tablets/Orodispersible Tablets (8 mg): 16 mg (two 8 mg tablets).
• Orodispersible Tablets (4 mg): 16 mg.
• Injection: 4 mg.

Pediatrics:

• >40 kg: 4 mg injection.
• <40 kg: 0.1 mg/kg injection.

Highly Emetogenic Chemotherapy:

• Adults/Geriatric/Children (12+ years): 30 ml (24 mg) oral solution 30 minutes before chemotherapy.

Moderately Emetogenic Chemotherapy:

• Adults/Geriatric/Children (12+ years): 10 ml (8 mg) oral solution 30 minutes before chemotherapy, then 10 ml after 8 hours, followed by 10 ml every 12 hours for 1-2 days post-chemotherapy.

Pediatric (4-11 years):

• Oral Solution: 5 ml (4 mg) 30 minutes before chemotherapy, then 5 ml at 4 and 8 hours after the first dose, followed by 5 ml every 8 hours for 1-2 days post-chemotherapy.

Oral dissolving film is to be placed on the tongue and dissolved without chewing or swallowing whole.

✔️ Interaction

Emeren 8 may cause a serious reaction when used with certain medicines like pain killers (acetaminophen, tramadol, ibuprofen), anti-anxiety (pregabalin, lorazepam), and antiparkinson medicine (apomorphine). 

The use of Emeren 8 is restricted in people with hypokalemia (low potassium) and severe liver disease. 

✔️ Contraindications

Contraindicated in patients known to have hypersensitivity to the drug or any of its components. Concomitant use of apomorphine.

✔️ Pregnancy & Lactation

Pregnancy:

• Animal studies in rats and rabbits (up to 15 and 30 mg/kg per day, respectively) showed no harm to the fetus or impaired fertility.
• No well-controlled studies in pregnant women.

Lactation:

• Ondansetron is excreted in rat breast milk.
• Caution is advised when administered to nursing women.

✔️ Precautions & Warnings

• Hypersensitivity reactions are possible, especially in those allergic to other 5-HT3 receptor antagonists.
• Not a replacement for nasogastric suction.
• May mask ileus or gastric distension post-abdominal surgery or during chemotherapy-induced nausea and vomiting.
